Presentations involving the liver, biliary tree, pancreas, intestine, or feeding. Reasoning splits early on what compartment is failing (hepatocyte vs cholangiocyte vs enterocyte vs exocrine pancreas) and how acute the picture is, because a few of these entities (biliary atresia, classic galactosemia, tyrosinemia, acute liver failure of any cause) lose their treatable window in days to weeks. Many GI presentations are the visible end of a metabolic, immune, or syndromic disorder, so a "GI" diagnosis often shifts category once the work-up is complete. Stool character, jaundice timing, and feeding-trigger history are the bedside facts that drive the next test.
